Dr Wagner values his patients' time, so he makes every effort to see patients at their scheduled time.
We ask that you provide us with enough notice if you are running late for an appointment so we can adjust our schedule accordingly. If you want to cancel you appointment, please give at least 24 hours notice so we can schedule for another patient.

What to Bring to Your Appointment:

* Your completed intake form and records request form
* Insurance card
* Other documentation you may have, i.e.: chart notes, lab results
* List of prescriptions, plus any supplements or over the counter medications

Your Cooperation is Appreciated

Naturopathic Medicine requires the patient make the greatest effort to pursue optimal health.
It is a cooperative effort between the patient and the physician. A thorough history and examination is often required to more thoroughly understand the patient's health. Frequently this can provide sufficient information to the physician to pursue treatment goals, other times more investigative laboratory tests yield better information. Dr. Wagner will order diagnostics that would be prudent to help evaluate your case. Some labs recommended may or may not be covered by your insurance. Dr. Wagner will discuss the options available to help best guide you to optimal health.

Insurance and Payment information

Insurance co-payment and supplements need to be paid for at the time of service. Insurance companies will be billed directly unless you direct us to do otherwise. For insurance coverage, please check with your provider. Outside labs ordered which are not covered by your insurer must be paid at the time they are ordered. Any balance remaining for office visits will be billed to you.
